Your form notification is configured correctly and emails are being delivered based on our logs. Here's how you can view your form email history: http://www.jotform.com/help/293-How-to-view-all-your-form-Email-History.
You have a custom email address as the recipient of the form. It is most likely that your service provider have blocked the emails from Jotform or the being filtered as spam. Kindly check that. If this is the reason, whitelisting us should help you fix it permanently. Here's how you can do that: http://www.jotform.com/help/145-Whitelisting-JotMails-IP-Addresses.
