How to change the subject of the email notification?

I reused a form, and renamed it, but the submitted responses that I get in my email are still titled with the name and year of the original form.

When they come into my email, they still say 2014 State of the Lake.  Where can I get it to change that?  Thanks, Mark

The reason why this happens is because the subject of your email is not changed.
