Form reverted to the original template used to create form. How to get it all back?

I created a form that I have worked on and saved for the past several weeks. Today the form reverted to the original template while I was previewing the form. 

Is there any way I can get that form back? Is there a an issue on your end. Things suddenly became very slow to load.

You can revert to any changes by taking a look at our revisions tool: How to View Form Revision History

Please take a look at it and let us know if you experience any issues restoring it, but per my knowledge there are no issues that are reverting forms back to what they were before.

Please note that if someone else has access they might have reverted the form using the same tool described in the guide above.
