We have created a form to be used as an application to summer program.
One of the requirements is that students upload a short video. Until recently this was working fine.
Now students report that after they add and upload the file they are prompted to upload the file again. As the video is a required part of the application, students are currently unable to apply for our program.

Unfortunately, I'm unable to replicate the problem. The students might be uploading invalid files or the uploading process was interrupted due to a connectivity issue.
Here's my screenshot with the test submission, you can check your submissions page to see my uploaded video file.
I would recommend that you use the default File Upload field and replace the Kloudless File Upload and see it works better for your students. Or you could also try other Video widgets that we have here: http://widgets.jotform.com/category/video.
See if this solves the problem. If you get the error in your end do let us know.
