Why I'm unable to see a checkbox in my terms & conditions field?

Hi there, 

The check box refuses to appear when I use this widget - does it have anything to do with the fact that I'm using Chrome? The link works, the label appears, the text appears - just not the check box. What can I do to fix this please? It's a little urgent as I kinda need to get this form out today... :(

The reason why you don't see a checkbox is because you're using an HTML field.

I suggest you to use terms & conditions widget instead.
