I urgently need a form backup and all your submissions made.
Do you mean to say that the you've made some changes in the form then after you saved it, the form fields are gone?
Did you see an error while saving your form? Please try checking your form revision history: http://www.jotform.com/help/294-How-to-View-Form-Revision-History. Please let us know if you can't see the changes in the revision and we'll try to forward this thread to our developers.
Just a note when making changes, you should always make sure to keep a backup of your existing submissions. Or, clone the original form and do the changes to the clone version. This way, the original form and its submissions are safe.
