How can I create a weekly sales call sheet

i want to make a form that can breakdown visits by day for the week.  A column for the customers name , notes about the visit and a place to upload a picture of our displays in the store.

If you want to add a dynamic content to the form, you can do that using our widgets such as the Configurable List Widget: https://www.jotform.com/help/282-How-to-Set-Up-the-Configurable-List-Widget.

For adding an image in the form, you can use our Image field under Form Tools section.
