    Only the top half of my form is visible

    Even on Jotform, when I click on the "preview button" only the top half of my form is visible.

    Ditto when I embed on my website.

    Hi jacktsai,

    Thanks you for posting your concern. Could you please try to use the Iframe embed code when embedding your form into your website? Please try it and let us know how it appears on your web browser.

    Per your instruction, I have switch over to iframe.

    Still display on the top part of the form, then a big blank space (presumable where the rest of the form fields should be).

    Just a reminder that the same issue is happening when I click on the "preview" button on the Jotform site (on the form edit screen) -- so I'm assuming that regardles of the method selected for embeding on my site, the display issue will still be there.

    Hi jacktsai,

    That is because you have conditions set in your form. The lower part of your form appears only when certain conditions are met. For example, the Deliver Option fields change depending if the option selected in the drop down menu is "Mail to you," "Mail directly to recipient," or "Leave for pickup at reception."
