Not sure what's going on. I've been using jotform for a while and never had this happen. The only thing that I added recently was picture ID and a captcha. I have removed those in hopes that that was the problem.

Based on what you have mentioned and the code provided in your post, I believe that the issue is not with the jotform, but that there was a change on your website that is now including the code that does not allow the jotform to fully / properly load.
To resolve this, please re-embed your jotform using iframe embedding code.
