On my Norcalcruises.com passenger information form, I have the address field near the top of the form, but the email sent does not include the information, nor does the submission when I look at it online. Not sure what the problem is there.

Unfortunately, in neither of your jotforms am I able to replicate this issue. Is it still occurring on your end?
