How to pass data in the name field to another form

Dear Sir

I have created a fault report form.

However, currently, the user has to repeat inputting his/her name and school/division each time he/she submits a new fault.

What would be your advise to save this repetitive process?

The user and school/division is fixed for each user accessing from his/her phone browser. Thanks!
