This form: was linked from an ad claiming to have been posted by AppleOne Employment Services. It was not posted by AppleOne. AppleOne never requests money from the applicant as this form is requesting. This appears to be a fraudulent use of your tools, and you should inactivate the form as soon as possible.

    Thanks Will.

The offending form & user have been suspended.

We do have software that automatically detects and automatically suspends forms that are clearly being used for phishing, but unfortunately some do get through the net.

We are very thankful to people who do bring suspect forms to our attention, we work extremely hard to stop JotForm being used in this manner.
