I created a form and now I can't find it in my account.
My user name is KDEE and my email is email@example.com
Here is the link for my form. Clients filling out the form are having problems when submitting the form.
After clicking the submit button the form hangs at "Please Wait" and never submits.

Upon checking the form #42003648890152, I found that it is not in your account but associated with another account with username ""kdsr". The email address used in this account is also "firstname.lastname@example.org".
Since both the account as same email address, do you want us to transfer this form to your account with username "KDEEE"? Do get back to us on this and we will do the needful.
I also tested your form and did not find any issue with that. It seems to work fine and I was able to submit your form without any problem. Please check the screenshot below:
