Creating a survey

Good Morning. I work for an Orthodontic practice and we are looking at sending out surveys to our patients after their appointments. I am wondering if the templates are able to automatically pull patient information from our practice management software (i.e. name and email). We currently use Tops Ortho for our practice managment software. If you could please contact me to let me know my options that would be great. Thanks!

If your management software is capable of exporting your patients information to an Excel or CSV file format, you can create a contact list form and import your patients information list using the generated file on our import data app.

Then, you can create your survey form and send it to your patients using our newsletter app.
