I have a 6-page form for which the submission period is now over. I want to update the form to include some post-survey information (e.g., prize winners). I'm trying to figure out the best way to do this without destroying the old form.
What I've ended up doing is cloning the existing form and editing the original form. However, if I remove the that now separates the form from the entries (in that the entries are attached to the old form but there are no more form fields). This isn't a big issue because the data is intact and I can go back to the form fields in the backup form if I want to use them again.
I'm also wondering if a second approach would work. I want to ask about it instead of using on a live form in case it causes issues. Here's how it would go:
Say the existing form's address is http://form.jotform.com/form/123, which was the default URL given to it from the system. (I know the address should be 11 digits, but I'm using this as an example). Can I then:
1. Create a new form that just has my post-survey information in it (i.e., no form fields).
2. Change the URL of the old form to something else (e.g., http://form.jotform.com/form/234) as long as it isn't currently being used.
3. Change the new form to http://form.jotform.com/form/123 to replace the old one.
This would keep the old form responses together with the old form fields and the users would see the new content on the same form page. The old form would no longer be live because the nobody else would know the URL. I'm just wondering if there is a problem renaming a new form to a JotForm URL that has already been auto-generated by the system and used.

I don't think this is possible in JotForm, each form has their Unique ID's, which simply means it can't be reused once generated. You can only replace your form's direct link by using our Custom URL feature but it doesn't mean you can now point it out to the OLD form URL.
Just to make it sure, I have this issue raised to our developers for further clarification on whether this is possible or not. They will respond to this thread as soon as they can. Thank you and have a great day!

You cannot change the URL for a form. You can clone a form and but then you will still lose the data.
If you would like to post a message on the form and hide the old questions, you should probably add a page to the beginning of the form and then use conditions to hide the second page. The condition might be something like this:
If "one of the questions" equal to "some random text"
Hide "Page Break"
I have tested this and it worked.
Here is what the form look like:
