My form (http://www.harmonixgolf.com/PO.html) will be filled out by new client.
I wish to capture the clients email address from the form to forward a copy of the form back to the client AS WELL as sending it to my for processing.

If you would like to send a notification to the person who filled out the form using the email address field, you can do that by creating an autoresponder, here's a guide on that: http://www.jotform.com/help/26-Creating-a-Form-Autoresponder.
