Email notification is not sent in the expected format

    Hi, I cloned this form: Application for employment'', and made some changes, I filled it out on the website as part of testing the form, and I got the shown below. it was absolutely different than the one i customized. i actually got the results for the actual template form that i used before customizing.

    How can I get results for the customized form that i want it?

    I took a look at your jotform (http://www.jotform.co/form/50658336563865) and I see that it has 2 email notifications.

    Please take a look at them and remove the one that you do not want to receive and only use the one of interest.
