I see that there is an active account (seveland) with Premium subscription plan and didn't see any issue with it. Also, I don't see any recent modifications to your JotForm in this account. If you are referring to some other account, please let us know. It would be helpful if you can provide us with the username or registered email address or Form ID to locate your account.
