Agree to terms and conditions check box

Hi, I would like to add a single checkbox that says "I agree to abide by the above terms and conditions.", and users have to click on this to be able to submit the form. Can you please advise on how to do this? Thanks!

You must be looking for the Terms & Conditions widget.
