Do you mean you have made a submission or are you currently working on some issues with your form? If this is related to a form, can we have the Form URL so we can look into it? Thanks for getting back to us. Are you referring to a form you submitted for a sponsor? Jotform is an online form provider that allows users to create and use forms online. We recommend reaching out to your sponsor directly as we are not affiliated with them. Jotform simply provides the forms to be used. If that's not exactly what you're referring to, please send us a screenshot of the document you have submitted or the link to the form so we can further check?
