I had a form designed by a CSS programmer. He used one of the radio button widgets.
They have now disappeared? (well they kind of 'half load' then disappear completely)
Your first though must be that there is a problem with my individual form, but that form has be duplicated 6 times on my account and is on different landing pages of my site. It has disappeared on all forms all at the same time. My guess is the associated widget??
By the way I never mess with the code, as I know nothing about CSS
If you could look into this asap it would be much appreciated.
P.s I'm not sure of the particular widget, but I think it was called line widget.

We have updated some of our widgets and I'm not sure if that is causing the problem. But refreshing the widget will fix it:
You will have to do this in the form builder, so you need to edit the form.
Lastly, it's the Line Checkbox widget.

I cant seem to see the refresh button on my editor? Is there another way to refresh?

Unfortunately, I just found out that there are options that have been removed in the widget that might cause this to happen. Here are the widget options after refreshing it:
And looks like the Refresh button is only available for the support team for now.
I've cloned your form and fix it, here's the new look: http://www.jotformpro.com/form/50695158440963. Clone this back and use replace the form that is currently on your website.
I have also submitted this thread to our developers so they can take a look further.

Hi, refresh button is not yet available to normal users, however we will enable this soon. For now all you have to do is to reintegrate the widget to your form and that will resolve the problem you were having.

Hi I appreciate the help.
As I run an online business based on filling out the form it has literally stopped my business in it's tracks.
Is re-intergration of the widget an easy process like the refresh button?
Is it possible you you to go into my form and just hit the refresh them? there are 5 forms highlighted with a star. If you could refresh those one it would really help.

The styling has all disappeared though.... i'm guessing I will need to get that re designed?

Yes, since the widget has changed Ben, you might need to have someone - or try yourself - work on CSS code.
