Thanks for the detailed summation of your problem. Before I forward this issue to the developers, I notice from the email logs that its possible you have recently changed from using the form users Email address as the Sender of notifications to email@example.com . is that correct?
Are these emails not getting through either?
Our mail servers are reporting them as accepting by your email host.

We apologize for any inconvenience this may have caused. I've checked your form and saw that you've already changed the sender address to firstname.lastname@example.org. Thanks for helping us. Can you please tell the person to add these ip address below to her mail provider's whitelist.
