    Adding conditions in a form

    Conditional items are included in the Follow up Survey Form template.

     

    Also, can you have the FAQ's open in a new window? Sometimes we need to copy/paste information from our forms, but need to go "back" in order to get the information.

    Regarding your question: How do I include "conditional" items on my form?

    This guide (http://www.jotform.com/help/57-Smart-Forms-Using-Conditional-Logic) will help you how to use conditions in your form. It will show you how to add a condition and it'll also explain the function of the conditional logic available.
