How to create conditional dropdown list

How do i create a conditional drop down list?

i.e. when user selects xx option, he will be presented with a certain info / image / text.  

We are trying to create an emergency evacuation form - whereby you select your current location and is then presented with info on your designated assembly pt.  Tks!
