I have been struggling to get my form integrated with google doc. So, I made a new form with only a name field to see if I could integrate a very basic form.
I get an error when integrating, but then when returning, I get that it is integrated.
When I use the link in the integration, it goes to my google spreadsheets area, but the spreadsheet or folder is not there.
I noted that the link seems to be missing something -- It is in this format but the key= doesn't have anything after the '='
What am I missing?

If you are getting an error while setting up the integration, it may not be completing. May I ask what the error is and at what point does it occur in the integration process? At the end of the integration it will provide you with a direct link to the sheet:
I am guessing that the authentication may be failing. In any case, here is our guide on integrating your form with a Google Spreadsheet:
If you have something blocking the popup for the authentication, that may also be causing the error.
I checked my test form to make sure it was still working and as far as I can tell, it indeed is working as expected.
Try the integration once again, make sure each step in the process completes and let us know where it fails if it does.

The integration is showing as integrated, but the link to view the integration and the actual file in google docs is not formed.

In that case, you should re-integrate the form to Google Spreadsheet.
Click "Remove Integration" button to remove the integration then do the steps from this guide to re-integrate:

I have removed and reintegrated this and one other form I have probably about 10 times, with the same result.

We can submit this thread to our developers, but can you try it again in the current form? Let's see if the integration fails again. So far, I cannot replicate it here. I've tried it to the clone version of your form:

Tried again, one last time. Still showing that the integration is ready, but link is not taking me to a excel sheet. Folder and sheet are not in google drive.

I checked your form and I can see that the spreadsheet link is broken indeed.
I cloned your form and I too was not able to replicate this issue.

It was observed that Google might have had some issue which must have interfered with the integration earlier. Can you try this once more to see if the outcome is the same. We will await your response.

Though we are unable to reproduce the issue, I have forwarded this to our development team to have a look. Our apologies for having you try to re-integrate your form with Drive several times, we just wanted to make sure that there wasn't something we were missing.

Someone posted that the problem may be that I am on Google Apps. Yes, I am on GoogleApps.
I will have our administrator enable GoogleDrive API and let you know what happens.

This is most true, Google has recently updated its user policy and now Drive API must be enabled for all Google App accounts to be able to use Google Drive and Spreadsheet integrations.
We will be working on handling those changes.
