can you tell me how to attach a form to an email so that a collegue can review?

If you want to set submissions notifications, so the data of an already filled form is sent to your or someone else to be reviewed, please see this guide: https://www.jotform.com/help/25-Setting-Up-Email-Notifications

Once you set it up, you can simply add another email address, into the recipient field, each followed by a comma and a space. Do not place anything after the last address.

Here is an example:

user1@domain.com, user2@domain.com, user3.domain.com
