Can't find the form I created!

I created a form and linked it to facebook. I can logon to facebook, access the form, fill it out and receive a message telling me I received a form. But when I login to my account, I can see the submission for but cannot find my actual form that I created. Can you help me find it please???

I am not sure what you mean by " But when I login to my account, I can see the submission for but cannot find my actual form that I created." You can only go to your form's submission page if you see the form in your account.

This form is actually an untitled form and that is the reason it is not displaying any label. Please check the screenshot below:
