Embedded form is not working correctly

Both forms work perfectly in preview mode however when embedded into Big commerce they both loose some of their functions such as - 1. 'Print form' does not work. 2. 'Click here for additional product listing table' does not open to show the hidden table. 3. The background is not highlighted when filling in parts of the form. 4. 'Date goods were received' field should only be visible if 'Have you received the goods?' field is 'yes' (it is showing permanently.) 5. None of the 'this field is a requirement' error is showing (the form can currently be submitted with no fields filled). 6. When the 'submit' button is clicked on the lower form, the 'how did we do?' form, the page jumps to the top of the page. How can I stop this from jumping so the viewer can see the 'thank you' without having to scroll all the way down the page again?

I see that you're using other JavaScript libraries on your site such as JQuery that might be causing this behavior in your form. Please, try to re-embed it using the iframe method instead.
