Why are my forms disabled?

    Everytime I try to visit my newly created form, it says it's disabled. But here on JotForm it says it's enabled.

    Your forms were disabled by our anti-phishing system because you're requesting sensitive information such as usernames and this is against our terms of use.

    We need to know more details about why you're requesting such information in order to evaluate if we can re-enable your forms or not.
