    Can I create this form on Jotform

    I expect to buy EverWeb for my web site.
    I need a form as we already have at

    Unlike other web forms, we require that customers print and sign ours. 
    They then fax it or scan and email to us.

    The form also needs radio buttons as you see so users can select among various items and specials.
    I have a new Mac and so need Mac-compatible apps.  The current site was created on a PC.

    All forms created in our solution are compatible with most (if not all) browsers for both PC and Mac so compatibility will not be an issue. You can even make your forms mobile responsive with the use of our Mobile Responsive Widget or enabling options in your form settings.     Our forms facilitate both submission and printing features so this will not be an issue also. If you would like your users to print their form before submission, you can simply provide the necessary instructions on the form and add the printer button by following this guide:  https://www.jotform.com/help/24-Print-Button-on-Forms

    You can even hide the submit button on the form by injecting the CSS code below:
