How can I create a Membership Card, sent in the form of a unique QR CODE

i am looking to create a membership form for dancers, dancers must all become a member. Each dancer will be emailed a type of membership card with a unique QR CODE. The QR CODE will be scanned at events and should show their unique membership number

This is the jotform from that thread: http://form.jotformpro.com/form/50066499796977 and you can see it working once you add your own email - which will send you QR bar code to your email.

Now the sample is based on email, but I only want you to see if that could work for what you are after. If it works we would need additional info such as what payment processing tool(s) would you be using on the jotform and if the number must be of specific format?
