Can I make this form in Jotform?

    I have attached a screenshot of what I am trying to create... is this possible?

     

    Form must be able to total costs on the box links and have different shading in the sections as shown.

    Form also needs to print exactly as shown.

     

    Tall order... and I have no idea where to begin although I have used JotForm for years now.

    Top right section is a logo... everything else is currently in text format.

    Entire top section could be a photo if this is easier.

    With our tool it is possible to develop a form of this nature but I am not able to guarantee that it would be an exact replica of the image you provided above. In my spare time I will devote myself to assist you in this form's development.

    It would be a good idea to use an image for the top section of the form because that would definitely make things a lot easier for you. Did you check our templates page to see if there are any available templates there that you can try or start out with?

    I will try to come up with something once I have some free time on my hands. So you will be updated on that via this thread. If you happen to locate any template that you would like to use/start out with, please let me know and I will help you with the necessary modifications.

    I should also add that it is possible to have multiple different colors to the background of each field with the use of custom injected CSS. This will not be of any issue. We will definitely walk you through the necessary steps to achieve this or if you wish you can follow this guide for the steps to have your CSS injected.

    Would the new online PDF form work well for this project?

    Still giving me the ability have the layout I am requesting?

    The Fillable PDF form will replicate the exact design for the form you designed in the Form Builder. So if we can achieve the exact design you provided above, creating the fillable PDF version of the form should look just the same.

    Just to add, conditional logic statements (such as show/hide) will not function in these Fillable PDF forms yet so they will just have the same design/layout but not the functionality.

    Can they still use the totals and signature sections?

    I did a test just now and I realize that the signature widget was not included in the Fillable PDF file when created even though it was added to the form. I can also see that the form calculation widget fields were not shown. I have created a thread to have this information passed on to our development team. 

    So, basically, the fillable PDF form will only allow your users to add information to text boxes, check boxes, radio buttons among other fields but base on my checks so far, it seem as if there is an issue with widgets. The fillable PDF will not provide automation/animation. It is merely available to collect information.
