How can I embed a Fillable PDF on my website to handle my form submissions?

This is all "wonderful," but how do I and where do I enter it on MY page so whoever needs it can fill it out to be sent to us?

Can you sent out a step-by-step guide?

Also, when this will take final effect?

Also, I have two sites/pages that use the present form.

    1) http://www.mspuppetry.com/MPGContactForm.htm

    2) http://www.mspuppetry.com/Puppet%20Arts%20Theatre/PATContactForm.htm

    Thank you ...

Fillable PDFs are intended for offline use. So, these are not forms that would be embedded on your website to handle form submissions via the web.

These PDFs will allow your users to download and complete your form offline. They can then have the form filled out, saved and submitted to you via email or have it printed and scanned.

I hope this answers your question. Do let us know if further clarification is needed.
