Why is my account suspended again and what can I do to fix this. I was not notified, But when I went to log on I was told my account is suspended

Unfortunately your IP address is associated to a lot of banned/suspended phishing accounts in JotForm.
This was the reason the phishing filter continuously disable and suspend your forms and account. The system was simply reacting to what it perceived as a phishing account.
I suggest you try changing your internet IP address to something different than what it is now to avoid getting banned by the phishing filter.
I have re-instated the account and forms again. You can check by logging to http://www.jotform.com/login
