I can't find my form. HELP!

I created a form called: "StrawberryWare 12.0". It seems to have VANISHED.  Can you tell me where it is or how to find it?  Thank you.

I checked your account and I was also unable to find a form by that name.  I checked for any forms that may have been created under Guest accounts that are associated with your IP address and I was also unable to find any.  There were also no forms in the "Trash" for your account so if it had been deleted, it was also fully purged.

If you possibly created it while logged in with a different email address or if you have a link to the form that is missing, let us know and we will be happy to see what else we can find.
