Unfortunately we can not assist you with this since we are not connected to StudioPress - Prose Theme developers, but you should be able to contact them about this if you have purchased the theme from them.

They would know exactly what you should do to embed the jotform, while we can provide you with the code to do it.

I would recommend using the iframe embed code: How to get the jotform iFrame Code

When you ask on their forum, you can tell them that the code is based on the iframe and script part and if needed, you can remove everything after <script> tag so that you only have iframe code ending with </iframe>.
