custom ccs forms

I want to use the source code and create a custom form look. Do you have recommendations of people of services that can do that.

You can actually inject CSS directly to your form:

You can also customize the look of your form using the form designer:

http://www.jotform.com/form-designer/

As far as recommendation for someone who can do this for you, a local web designer would probably be your best bet.  Someone you can meet with in person is always better than fully communicating online.
