The form shows only about 20 percent of it, then it takes about 15 seconds for the rest to load?
Also, the phone number section, even though on the preview the masking works fine, in the website theres not masking, it types the numbers like this:
where as in the preview it does it like it should:

I initially was able to reproduce the lag issue when viewing the jotform on your website.
I found that it was due to the wrong URL link used for the background image used in the form.
I have fixed the issue in the injected CSS code in your form that uses the wrong link to the image.
Please checked again in your website if the loading issue still happens. As of this writing, I checked and could not reproduced it anymore.
Just to be sure, clear also your browser's cache before testing/checking again.
Let us know if the issue persist.

I forgot to mention that Phone number Input masked was also working when I test.

Yup, it works now. I kind of figure something was not loading due to my bar at the top getting stuck at 80 percent, but i couldnt figure out what was causing the issue. Thanks for your help! After this fix the masking is now functioning as well. :)
