I added a responsive form on this page: http://legearpelling.com/about-us-andrew-tablotney-president/
But on my mobile it is not working fine on my mobile as you can see in attached screenshot.

Please get me solution as soon as possible.

Please let me know how to solve the calender issue so that i do not see the calender is cut off in screen of my mobile.

Hi,
Have you check this guide on how to enable mobile responsive to your form here: http://www.jotform.com/help/311-How-to-make-forms-Mobile-Responsive. See if the form becomes mobile responsive after that, if would be better if you could check the direct link of the form.
Another, I noticed that the actual website is not mobile responsive? Which means the form will be viewed on how it is embedded in your website, if I am not mistaken.
Another thing that you can do is set the form's width smaller for the calendar field to adjust. Or you can use a separate field for the Time instead, have it on a separate line.
