How do I get my text area to show on the downloadable PDF version of the form?

    Hi there!


    I made a model release form pdf to embed into my wordpress site. I did this part fine.

    The client fills out basic info : name, date, address, etc.

    I added a "text area" to the form which is the wording/meat of the entire model release.

    I then added a signature field at the end.

    The problem is: when I review the submission or export as PDF - it just includes what they filled out. It doesn't include the actual information in that text box of the legalities of the model release. I want to be able to save these with the content the client is signing to but it defeats the purpose if I have a blank signed page with no terms. 

    I will attach a screen shot of what the final PDF looks like as well as the submission. 

    Please help!

    My form looks like this. I want all the actual model release info to be included in the form submission to keep it for my records. But it doesn't include any of that text on the saved submission. What am I doing wrong?

    You will have to check "Show Headers and Texts" in the form submissions page settings.

    a. View your form submissions page:

    b. Check the mentioned option:

    Note that you will only do this once in the form. After you check the option, text fields will be included in the generated PDF format of a submission.
