Hello,
The YouTube videos I have embedded in my form are not displaying properly. The same video is displaying multiple times, even though each video is different. The attached screenshot is how my form displays in the editor.

Hi,
I can't seem to find the cause of the problem. I've cloned your form and all videos are showing correctly, here's the link: http://www.jotformpro.com/form/50776945808975. I have also created a new form and added the videos to it using the widgets and they are still showing correctly: http://www.jotformpro.com/form/50776619895979. Please try to re-add the videos, see if that will work.
