Form fields are required but in submission emails most fields are blank.

    Also my client says that even though I have all fields set as required when he receives the email most fields are blank.

    This is happening because at some point the data table of the notification was edited. The data table is that portion of the notification headed by Question and Answer, as indicated below:

    I know it was edited in your form's case because the words "Question" and "Answer" have been replaced with dashes:

    If you compare the screenshots above, you'll notice that many of the tags don't match. Those in the first screenshot are correct.

    The solution would be to either replace the notifier with a new one, or if you don't wish to retain the current one, use the Form Fields menu (see below) to replace the tags.
