I rec'd am email saying the issue was fixed. I tried again. Same result. PDF form maker not working.

I've tried creating a PDF form of your JotForm and I was able to open it using Adobe Reader XI without any issue.
What version of Adobe reader are you using? I've also cleared the form caches to see if that helps. Please try clearing the browser cache and try creating the PDF form again. Please let us know if the issue still persists. We will be happy to look into this issue further.
