I'd like to require passcode access to my site

I want to require a passcode to access my site. But, Jotform seems to be preventing this. I am rewriting my form from another form site.  On that site I have been able to require an access code before users could access my form.  Can I do that in a straightforward way on Jotform? Please show me how I can do that. Thanks.

p.s. I have included the URL to an actual live form that requires a passcode. The passcode is:   HL1401.  (It IS case sensitive.)

    Hi,

Creating login forms or collecting sensitive information is not allowed through JotForm. However, you may protect your JotForm by hiding the fields and only to display them upon entering a secret code. This field can be labelled as access code or authorization code. The secret code can be validated by using our conditions wizard.

Hope this information helps! Please get back to us if you are referring to something different. We will be happy to help.

You are very welcome, we are here to help!  Do keep in mind to stay away from using phrases such as "password" or "passcode", using these in your form will most likely result in your form being suspended by our system.

    Here's my form URL   http://www.jotform.com//?formID=50738228165155

I use the term "Authorization Code" and it doesn't seem to work.  It worked at first. But, now it doesn't.  Can you please help?

Please fix a condition #84 on your form. It has one incomplete conditional rule which is causing the conflict.

Thank you.  I did what you recommended and it worked.  I appreciate your help.

But, there is another problem.  When my user enters the Authorization Code it works perfectly...(the FIRST TIME). But, when the form is reused, the Authorization Code is ALREADY ENTERED in the field from the last time that it was entered.  How do I cause the field to be erased upon completion of the form?
