The Google Sheet is created but form responses are not populating. The form is titled "KPTI Micro-Tools Regrinding Services Survey". Please help...thanks.

Can you please remove the integration and rebuild it again?
Just look for the spreadsheet integration, click on it
And click on the remove button

Thanks for the support. I removed and reinstalled the integration. Responses are being populated for Name, Company, Email, IP and Submission ID but not for all the other fields.

Our sincere apologies for the inconvenience caused.
Please try to re-integrate the form again to Google Drive and if possible, share the spreadsheet link to our support Gmail account email@example.com so we can check and test it.
I have cloned your form and I can't replicate the problem for the few entries that I've made in the form.
I look forward to your reply.
