Button Stopped Working

    Hi...using a simple sign-up form w/button on our website and the button stopped working. Not sure when or why but was just notified today. It looks like we have a huge gap in submissions between the end of January and now. The button appears to work in the preview on jotform, but does not work on the website. See footer here: www.dermahut.com.

    This is the code used. I deleted what's on the website and re-created the code as well today and it still doesn't work. Please advise how to correct his problem.

    Rather than using the source code to embed your form, if possible try using the iFrame embed method instead:


    If the form works on JotForm in the preview but not in your site, there may be a conflict between one of the scripts running in your site and the scripts for the form.

    If this still does not allow your form to work correctly embedded, let us know and we will be happy to have another look.
