ALSO your website keeps crashing saying "jotform is not responding" I tried on 2 windows 8 laptops and a MacBook... when I try editing my forms...

The form is too simple and doesn't contain too many fields. Please try to clear your browser's cache and cookies for Jotform then login again. I was able to edit your form without any issues so it must a browser related problem.
