If I understand correctly you're receiving empty information in your forms, even though is filled. Is that correct? I couldn't find a form on your site, can you let us know where do you have it embedded or the URL of the form so we can check?

If my assumption is correct, you may try to re-create your notifier and also check if the empty fields are required.
