How do you get a description field?

    I am wondering how you get a description field in a drop-down or check-mark question

     

    For example:

     

    Which is your preference:

    [ ] Option 1: Please describe [TEXT BOX]

    [ ] Option 2: Please describe [TEXT BOX]

    [ ] Option 3: Please describe [TEXT BOX]

     

     

    FYI, I see the steps for creating a text box for 1 other field...i am wondering how you could do it for more than one option.

    Hello. 

    You can use Conditions to show Text Box fields if a specific option in a Check Box field is selected. 

    example:

    Start by setting up your form as below: 

     

    Now, go to the Conditions option in the Setup & Embed menu. 

    Create a condition to show the relevant Text Box for each option in the 'Options' Check Box menu. 

    e.g show 'Option 1 - Please Describe' if 'Options 1' in 'Option 1' is selected:

     

    Do similar for the other two options.
