If I understand you correctly, you are getting this error once you hit submit in a fillable PDF form right?
May I ask what program are you using in order to open this fillable PDF? We recommend opening these forms using Adobe Acrobat Reader or Foxit Reader.
I cloned your form and created a fillable PDF form, opened it in Foxit Reader and I was able to submit it successfully.
After clicking the "Allow" button, it was sent properly.

I don't have Adobe Reader, but I do have Adobe Acrobat Pro and Mac Preview. Now, when I submit the PDF in Acrobat Pro I get that Server error, and when I submit it via Mac's Preview, I get nothing, and no submission. I also discovered that if the PDF is opened in the browser's PDF preview (chrome), it does not submit. This could be an issue because what if, by default, on someone's computer PDF gets opened via browser's built in pdf preview?
Secondly, I created a simpler form and was able to submit it via Acrobat Pro. But the other form still gives me server error on submission.
And lastly, the PDF form removes validation (required fields). And doesn't even indicate with the red asterisk which fields are required. So it seems like the PDF Creator still has some bug issues that need to be addresses before I can go with it prime time.

I was able to recreate it to the fillable PDF of your last form ID: https://www.jotform.com/form/11652817531. The same thing happened when I cloned your form and get its fillable PDF format.

I don't' see any settings in the form that would cause this, and I also can't recreate the problem using a new form. I have now submitted this thread to our developers for further investigation. If there are updates, you should know it via this thread.

-Unfortunately, the browser PDF viewer will not work. You will have to use a software such as Foxit or Adobe Reader in order to submit the fillable PDF form. You have to advise the users to download Adobe Reader to submit it.
